Position summary
Northeastern Public Health (NEPH) seeks a Public Health Nurse to help support the Healthy Families Program by applying key public health strategies to support optimal preconception, pregnancy, newborn, child, youth, parental, and family health. Job responsibilities
- Conduct screenings and assessments with families and work collaboratively with the Family Home Visitor to deliver home visiting/virtual/telephone services as outlined in the Protocol;
- Connect families with community resources and programs and refer to health and social services, as needed;
- Support, advise, and consult with individuals, and community, and health care providers on a range of topics;
- Develop, implement, and evaluate local public health programs and services using a comprehensive health promotion approach to support healthy growth and development in the population;
- Consult and collaborate with internal and external teams to assess local need, identify priority population groups, and determine effective interventions;
- Provide health information and teaching;
- Other duties as assigned.
Qualifications
- College diploma or certificate in child development or early child education program, or related field;
- Previous work experience with home visiting programs or early child development services is considered an asset;
- Ability to communicate clearly and effectively, both orally and in writing;
- Fluency in both official languages is an asset;
- Ability to meet the physical and mental demands of the position;
- Ability to work flexible hours and travel within and outside the health unit catchment area as required;
- Must have access to a vehicle and a valid Ontario Driver’s license;
- Satisfactory criminal reference and vulnerable sector check;
- Required to comply with Immunization policies, including COVID-19 vaccination, unless a valid medical exemption is provided.
